Dr. Little prefers placing the incision in the crease under the breast, also known as an "inframammary approach". Implants can also be placed through incisions in the axilla, belly button, or around the areola. However, these techniques have been associated with an increased risk infection and implant misplacement.
After making an approximate 1.5 inch incision in the breast fold, the breast tissue and pectoralis muscle is then elevated to allow implant placement. Depending on patient preference, saline or silicone implants may be utilized. Implants are then placed either above (Sub-glandular Implant Placement) or below (Sub-muscular Implant Placement) the pectoralis muscle, depending on multiple factors. The patient is positioned upright during the procedure to ensure symmetry.
Implants are placed under the most sterile conditions and utilize the Keller Funnel. This is a device that allows sterile placement via a smaller incision than is traditionally used. Long-term complications and infections have been reduced by using the Keller Funnel. Once the implant is in place, the entire area is rinsed, the incisions are closed with absorbable sutures, and the area is bandaged
